If you have one of the above-mentioned commercial insurance plans, please notify me so I can send you a link to verify your benefits. We will need to confirm that I am an in-network provider for your insurance plan and that you have mental health benefits. Insurance benefits must be verified by my billing service prior to our initial appointment.
If you choose to use insurance for counseling sessions, please be advised of the following obligations that I am under per my contract with your insurance company as an insurance provider:
1. Insurance companies require a mental health diagnosis to qualify for services to determine if services are medically necessary. This diagnosis will be listed on all claims filed with the insurance company.
2. Insurance companies have the right to request session notes as a condition for payment. Upon their request, I am obligated to release confidential information about you and our sessions to your insurance company.
I file claims with the commercial insurance plans listed on this page.
All other claims will need to be filed by the client as an out-of-network claim. All out-of-network clients are responsible for the session rate of $150, due at the time of services.
